Telehealth provider InTouch Health and Dignity Health have expanded their partnership, extending InTouch’s existing services in Dignity’s acute care settings into post-acute and ambulatory facilities. With new software, tablet and care options, InTouch Health’s services can go to any patient location with a laptop, tablet or fully-integrated solution that is designed for specific conditions, which also allows for connection with diagnostic tools and multiple providers for each patient visit.

Mount Sinai Health System is now using Salesforce’s CRM platform to coordinate and manage care of Medicaid members within its Performing Provider System, which was formed in 2015 along with the overhaul of Medicaid. The Mount Sinai PPS covers more than 350,000 Medicaid members across the New York Metro area, and the collaboration with Salesforce will allow the health system to connect every provider and organization across the ecosystem and easily transfer information between each in order to better understand patient needs and provide personalized care.
"With Salesforce, Mount Sinai is arming thousands of Medicaid providers and community organizations with a more precise view of the demographics, lifestyle and health histories of patients," Dr. Joshua Newman, chief medical officer at Salesforce, said in a statement. "As a result, they'll be able to collaborate more closely and put their patients at the center of care."

Digital health company Mango Health, which makes a gamified app for patient engagement and medication adherence, announced a strategic investment from — and longterm partnership with — Express Scripts, the largest pharmacy benefit management organization in the United States. The amount of the investment was undisclosed. Through the deal, Express Scripts’ 85 million clients will have access to Mango Health’s mobile apps, which allow users to track their medications and health habits and encourage medication adherence through raffles for rewards. Care coordination technology company MedAptus is working with MD Interconnect, which makes a cloud-based mobile and web communication platform for healthcare workers. The partnership will integrate MedAptus’s patient assignment technology Assign, with RapidConnect, MD Interconnect’s secure messaging service. Rothman Institute, which is one of the largest orthopedic practices in the country, has expanded its existing relationship with Force Therapeutics to offer the technology company’s digital platform of pre-and post-surgical care plans. Rothman has used the platform for three years, accounting for more than 15,000 patients who have accessed Force’s interactive, video-based exercise and educational program. Forty-one surgeons at Rothman now use it regularly in their practice, and the organization expects that number to grow as they expand the Force platform into spine and sports medicine.

CityMD, a network of walk-in, urgent care clinics that works with partner providers, hospitals and health systems as well as offering self-service kiosks, has partnered with Montefiore Health System to improve access to primary, specialty and urgent care services of both organizations. Through the collaboration, Montefiore will be CityMD’s exclusive provider in the Bronx and Westchester, and patients can also benefit from the online appointment and follow-up features of CityMD’s platform. As 60 percent of CityMD’s patients do not have a regular care provider, the organization works to provide a continuum of care within its provider networks.
